North Simcoe Muskoka Diabetes Regional Coordination Centre

The Ministry of Health and Long-Term Care (MoHLTC) announced in October 2012 that the funding and accountability of the Diabetes Regional Coordination Centres and Diabetes Education Programs that were funded by the Provincial Programs Branch of the MoHLTC would be transferred to the 14 provincial Local Health Integration Networks (LHIN). For the DRCCs this transfer occurred on February 1, 2013.

Centralized Intake is now being done through the South Georgian Bay Community Health Centre. For more information, please call (705) 422-0900 ext. 119.

Person-Centered Care Guideline and Survey

The objective of this guideline is to establish a standardized set of recommendations for providing person-centred care. It is intended for use by patients (and/or family members and caregivers) and their care providers to inform the provision of person-centred care.

The video and accompanying survey are intended to introduce you to how our health system is striving to achieve Person-Centred Care. It should take no more than 10 minutes to complete, during which time you will be provided with some basic definitions and concepts, and have the opportunity to stop and assess what you've learned. By the end of the video, you will:
1. Build awareness of how Person-Centred Care is relevant to your role;
2. Better understand Person-Centred Care; and
3. Recognize Person-Centred Care behaviors and practices
